How Legacy Projects Honor a Life and Preserve What Matters Most

At Angel Hands Hospice and Palliative Care, we believe every life holds stories worth keeping. Our Legacy E-Book is a digital keepsake created from a patient’s stories, reflections, and photos — something families can revisit, share, and treasure for years to come. Legacy Projects are about more than remembering facts. They help capture the heart of a person — their humor, traditions, values, and the moments that shaped them.

What This Means for Future Generations

Long after the details fade, a Legacy E-Book becomes a family touchstone. It helps children, grandchildren, and great-grandchildren understand where they come from, what their loved one believed in, what mattered most to them, and what they hope their family carries forward.

Who Can Request a Legacy E-Book

This is available to:

  • Any current Angel Hands patient and their family

  • Family members of past Angel Hands patients

If you are interested in creating a Legacy E-Book for someone you love, reach out and we will walk you through the next steps.

Legacy E-Book Questionnaire

You can answer as many or as few as you’d like. Short answers are welcome. Longer stories are always appreciated.

Note before you begin: The form we send you will not automatically collect your name or email address. Please include your contact information in Question 2 so we can follow up if needed. The form does not save automatically, so if you need extra time, please send your answers via email to the Director of Volunteer Services. Fill out the contact form below and they will contact you.

The Questions We Ask

  1. What is the patient’s name?

  2. What is your name, email address, and relationship to the patient?

  3. Can you share a moment that always brings a smile when you think of your loved one?

  4. What advice or life lesson did they share that has stayed with you?

  5. Is there a specific place or event that was particularly meaningful to them?

  6. How would you describe their spirit in three words? (You may elaborate if you wish.)

  7. Did they have any unique sayings, jokes, expressions, or wisdom that were specific to them?

  8. What did they cherish most in life (family, friends, career, etc.)?

  9. Was there something they were passionate about advocating for or supporting?

  10. What is a moment you shared together that you’ll never forget?

  11. How did they make you feel loved or supported?

  12. How did they influence the person you are today?

  13. Please share any other special memories or stories you’d like included in the Legacy E-Book. These can be anything from childhood memories, life achievements, favorite activities, or any meaningful moments.

What You Can Send to Add Into the Book

Families are welcome to send any additional content they would like included, such as:

  • Pictures of handwritten recipe cards or favorite recipes

  • Important stories, memories, or milestones

  • Letters, poems, prayers, or readings

  • Photos (with names, dates, and locations if available)

  • Favorite sayings, jokes, traditions, or “things they always said”
