Summary

Qualifications

  • RN license (Texas or Compact State)

  • One or more years of clinical experience (hospice preferred)

  • Valid driver’s license and reliable transportation

  • Ability to work independently and adapt to changing conditions

Key Responsibilities

  • Conducts home visits and assessments

  • Develops, implements, and revises plans of care

  • Supports families with education and end-of-life guidance

  • Participates in IDG meetings and documentation

  • Takes part in on-call rotation as needed

Summary
Provides emotional, psychosocial, and practical support to patients and families. Assists with advance care planning and participates in interdisciplinary coordination.

Qualifications

  • MSW or BSW with one or more years of healthcare experience

  • Strong communication and interpersonal skills

  • Reliable transportation for home visits

  • Knowledge of local community resources

  • Completes psychosocial assessments

  • Offers grief, crisis, and end-of-life counseling

  • Assists with advance directives and resource planning
